Pre-Award

Early Proposal Notification Form
To notify Grant and Contract Personnel of a planned grant proposal submission.

Proposal Approval Form
The Proposal Approval Form (PAF) should be completed and submitted with all grant proposals at least 5 working days prior to the deadline, or with prior review, 3 days in advance.

In addition to the PAF, other forms may be required. Contract OGCS if you have a question about whether a form is necessary.

• NEWInvestigator Financial Disclosure Form
  Principal Investigators are required to disclose any significant financial interests that may present an actual or potential conflict of interest in relationship with a sponsored project. This disclosure is required for all proposal submissions.
• NEWChecklist for Direct Charging Administrative Salaries on Sponsored Projects
  Investigators are required to complete the Checklist to disclose reasoning for direct charging clerical and administrative salaries.
• Request to Issue a Subcontract Form
  This form should be completed and submitted by a Principal Investigator/Project director when subcontracting to another institution using sponsored project funds.
• Subrecipient Commitment Form
  This form is used to document the participation of proposed subrecipient/subcontractors. No subaward will be issued until the completed form is received.
• NEWF&A Reduction or Waiver Request Form
  The request for an F&A reduction or waiver should be completed and submitted to OGCS at least two weeks prior to the sponsor's deadline.
• NEWTuition and Fee Waiver Request Form
  The request for a tuition and fee waiver should be completed and submitted to OGCS at least two weeks prior to the sponsor's deadline.

Fringe Benefit Rate Calculator Updated to include new benefit rates!
The Fringe Benefit Rate Calculator is an Excel worksheet file that can be downloaded and used to calculate the fringe benefit rates used in grant proposals.

Pre and Post Award Sponsored Project Action Request

NEWSponsored Project Action Request (SPAR)

This form replaces both the OPAS and the At Risk forms and should be used to request changes or modifications to pending or awarded sponsored projects.
